How to enroll a subject into the system?

In On the main menu, user click on "Enroll" tab
Enter information for required fields:
General Information:
Study Subject ID: is generated by one of three ways
- Auto-generated and Non-editable
Person Secondary ID:
- User must enter for this field if administrator configure in building study
- This field is hidden if administrator does not configure in building study
Secondary ID: sub ID for subjectsub-ID for the Subject if needed. This field is optional.
Enrollment Date: user you can type manually enter or select from the calendar
Sex of subjectSubject: choose from the available options
Date of birthBirth: user you can type manually enter or select from the calendar
Subject group classGroup Class: to categorize Subjects within a Study, such as for by treatment options
User You must select subject group class for subject in case this is required
User can select subject group class for subject in case this is optional
User cannot see subject group class in case Study does not have subject group classa Subject Group Class for the Subject if this field is required.
You can select a Subject Group Class for the Subject if this field is optional
You cannot see the Subject Group Classes if the Study does not have those.
After entering data, user you have three options as below:
- Save And Assign Study Visit: Save subject the Subject information and schedule visit a Visit for subjectthe Subject
- Save And Add next Subject: Save subject the Subject information and add a new subjectSubject
- Save and Finish: Save subject the Subject information and go to "the Enter data" Data screen

Note: User You can add Note Notes and Discrepancy Discrepancies for some fields such as : Person ID, Enrollment Date, Date of birthBirth, and Sex. (Reference Refer to 14.Query.Discrepancy Note) |
