Log In to CheckPad

To access CheckPad, in your web browser go to the CheckPad login page . User enter username/on your web browser. Enter your username and password then click on "Login" LOGIN
Forget password

If user you forget your password, please click on "on Forget Password?" link
User fulfill Fulfill the information for the required fields then click on "SUBMIT PASSWORD REQUEST"
- Case 1: if user you enter correct information, the system send sends a successful message and send pass word to sends the password to the entered email
- Case 2: if user you enter incorrect information, the system displays a warning message so user that you can re-enter the information or contact with the Administrator
Reset password in the first login

You have to change the password in the first login. You enter Enter information in the required fields then click on "Change Password" CHANGE PASSWORD
- Case 1: After changing password successfully, user you can go to Homepage according to user's your role
- Case 2: If user you enter incorrect information, the system displays a warning message
If this function is NOT configured in buidling your study, user you don't need to change your password in the first login. User You can directly go to the "Change Study/Site" page directly.
Update "User Profile"
...
User You can update user's your profile " when click by clicking on "My Profile" shown as the image below image. Update screen is shown The update screen will then show up.
User can update:
- First Name/ Last Name
- Email address
- Default active study
- Password challenge question
- Update password: when user check on "Yes"
- Phone number
Then press on Select "Confirm Profile Changes" to save changes changes.
Select Site/Study

After loging in successfully, the system allows user to select site/Study study
After selecting the desired Site/Study, user you will go to Homepage according to user's your role.

Log Out

To logout, user make step as below imagelog out, follow the steps as shown in the image below.
